Adults

Adult Single: $90/mo

Adult Family: $85/mo

Children

Child (w/family): $15/mo



• Acute care visits only (wellness/managed care should be taken care of at a pediatrician)
• Primary residence must be under the same address as the Head of Household/Primary account holder

Military & Law Enforcement

Military or law enforcement: $80/mo

Discounts

$5 discount per adult, per month for multiple DPC members who live in the same household
